Location: Portland, ME | Part‑Time | On‑Site

Overview: The Per Diem Home Infusion Pharmacist is responsible for ensuring the safe, accurate, and timely preparation, verification, and dispensing of sterile and non‑sterile compounded medications for home infusion patients. This role supports day‑to‑day pharmacy operations on an as‑needed basis, including order verification, clinical assessments, and patient/caregiver education. The pharmacist functions in accordance with all applicable state and federal regulations, USP standards, accreditation requirements, and organizational policies.

This position will serve as the acting Pharmacist‑in‑Charge (PIC) during periods when the PIC is unavailable, including PTO, illness, or other absences. During these times, the Per Diem Pharmacist assumes full operational, clinical, and regulatory oversight as required by Maine Board of Pharmacy regulations.

We are seeking a dynamic and experienced pharmacist to support the current staff in day‑to‑day oversight of pharmacy operations, ensuring clinical excellence, regulatory compliance, and exceptional patient care.

Key Responsibilities: Pharmacy Operations & Day‑to‑Day Oversight

Oversee daily pharmacy operations to ensure safe, high‑quality infusion therapy services across therapies such as immunoglobulin, anti‑infectives, specialty biologics, nutrition support (TPN), pulmonology therapies, alpha‑1 therapy, and anti‑inflammatory therapies.

Direct and optimize pharmacy workflows to enhance efficiency, including speed‑to‑service and post‑therapy follow‑up.

Maintain a clean, organized, and customer‑friendly environment that reflects Vital Care of Portland’s values.

Support day‑to‑day operations on an as‑needed basis, including order verification, patient education, and clinical review.

Clinical Practice & Patient Care

Review, interpret, and clinically verify infusion therapy prescriptions for appropriateness, accuracy, and compatibility.

Conduct ongoing clinical assessments and therapy monitoring for home infusion patients.

Provide drug information and therapy support to prescribers, nursing teams, and patients/caregivers.

Educate patients and caregivers on medication administration, storage, handling, and safety requirements.

Ensure seamless patient care coordination in collaboration with nursing, reimbursement, and customer service teams.

Document all clinical interventions, assessments, and communications in the EMR.

Oversee and verify sterile compounding activities in accordance with USP

/ , ACHC standards, and company SOPs.

Perform final product checks before dispensing to ensure accuracy, sterility, and labeling compliance.

Ensure adherence to best practices in cleanroom workflow, garbing, beyond‑use dating, and environmental monitoring expectations.

Regulatory, Quality, and Compliance

Ensure compliance with federal and state Board of Pharmacy requirements, ACHC accreditation standards, USP

/ , and internal policies.

Stay informed of evolving laws, regulations, and clinical advancements in infusion therapy.

Uphold patient confidentiality and comply with HIPAA regulations at all times.

Support quality initiatives related to clinical excellence, safety, and documentation accuracy.

Inventory, Ordering & Supply Chain Management

Accurately dispense, order, and maintain inventory of medications and infusion supplies.

Identify and communicate potential medication shortages, incompatibilities, or substitution needs.

Ensure appropriate storage, handling, and system tracking of specialty biologics and infusion products.

Provider, Clinic & Partner Communication

Collaborate with referring providers, clinic administrators, and clinical staff to resolve prescription issues and ensure smooth coordination of care.

Provide clinical consultation to physicians and teams across Vital Care of Portland’s partner organizations.

Maintain positive professional relationships supporting continuity, communication, and operational clarity.

Mentor, motivate, and support pharmacy staff as needed to foster a collaborative, high‑performance culture.

Project a positive, professional attitude that reinforces Vital Care’s mission, vision, and values.

Assist in onboarding or training activities for new pharmacists or technicians, when requested.

Additional Duties

Perform additional responsibilities as assigned and consistent with Vital Care policies.

Participate in staffing coverage for PTO, high‑volume periods, or emergent operational needs.

May include occasional evenings, weekends, or on‑call duties; flexible scheduling with opportunities for expanded roles.

Maintain readiness to support urgent requests and adapt to changing priorities.

Qualifications:

Bachelor's or Doctoral (PharmD) degree in Pharmacy from an accredited university.

Active, unrestricted pharmacist license in the state of practice.

2+ years of full‑time retail, clinic, or home infusion pharmacy experience preferred, with specific experience in infusion therapies (e.g., immunoglobulin, TPN, or specialty infusions) strongly desired.

Strong knowledge of pharmacy operations, clinical protocols, and applicable regulations regarding infusion therapies, compounding standards, and infusion device operation.

Ability to work independently, manage changing priorities, and support urgent requests.

Exceptional attention to detail and commitment to patient safety.

Experience with home infusion software (CareTend, CPR +).

Familiarity with comprehensive drug formulary and specialty biologics.
