As an APRN, you will manage the medical needs of residents within skilled nursing facilities, build strong relationships with administrators and staff, and serve as the primary point of contact for emergencies and urgent care visits. Responsibilities
Provide primary, urgent, and preventative care for residents in assigned skilled nursing facility; may also care for assisted living residents. Verify admission orders, conduct initial assessments, physical exams, medication reviews, and depression screenings. Collaborate with the facility medical director, attending physicians, and nursing staff to deliver the highest level of care. Communicate and counsel family members on treatment options when there are material changes in status. Minimum Qualifications
Valid Nurse Practitioner license in applicable state. DEA license. National Nurse Practitioner certification. Clinical background in adult, family, or geriatric specialties. COVID‑19 vaccination required unless qualified for a remote, religious, or medical exemption. Preferred Qualifications
Experience in internal medicine, gerontology, or cardiology. Excellent administrative, organizational, and communication skills. Ability to work independently. Detail‑oriented, dependable, and reliable. Additional nursing experience in LTC/SNF is a plus. Location
