Your Role As a Service Facilitator (Direct Support Professional), you provide support and supervision to individuals with intellectual and developmental disabilities within a residential setting. Working with the House Supervisor, you facilitate day‑to‑day services for residential consumers, empowering them to take ownership of their home, participate in the local community, and learn independence skills.
Responsibilities
Develop rapport with consumers and cultivate a safe, supportive relationship.
Provide supervision and guidance during skill‑building activities.
Document implementation of goals, objectives, and service plans.
Maintain daily consumer case notes.
Oversee consumers’ medication.
Assist with daily living activities, including meal preparation and personal hygiene.
Transport consumers to activities and appointments.
Maintain a clean, safe environment, assisting in household chores as needed.
Qualifications
High School Diploma or GED recognized by the Illinois Department of Human Services.
Preferred: One year of experience working directly with individuals with developmental disabilities.
Valid Illinois driver’s license with a clean record (no violations in the past five years).
Demonstrated 8th‑grade reading level per standardized testing recognized by the Illinois Department of Human Services.
Completion of all required DSP training within 120 days of hire and ongoing certification maintenance, including medication administration.
Willingness to work in other CILA homes as needed to provide quality care.
