Job Description Center for Human Development (CHD) seeks a full‑time Psychiatric Mental Health Nurse Practitioner for our Appleton Clinic in Holyoke, MA, as we expand our outpatient continuum of services. CHD outpatient services include individual counseling, psychiatry, primary care, crisis services, recovery coaching, case management, treatment for substance use disorders, and peer supports.

Essential Duties

Provide outpatient psychiatric care including psychiatric evaluation(s), assessment, diagnosis, and treatment of adults and youth.

Collaborate with physicians, advance practice nurses, psychologists, social workers, nurses, and other professionals.

Provide clinical consultation in person or remotely to other members of the clinical team.

Collaborate with guardians, relatives, and significant others of people served when clinically indicated.

Provide counseling to people served and their families when clinically indicated.

Document evaluations, assessments, diagnosis, and treatments in electronic health records.

Participate in individualized treatment plan designs.

Order, review, and interpret laboratory and other diagnostic tests.

Qualifications

State certification as a Psychiatric Mental Health Nurse Practitioner or Clinical Nurse Specialist.

Unrestricted license to practice as an Advanced Practice Registered Nurse (APRN).

Unrestricted DEA registration.

Massachusetts Controlled Substance Registration.

Registration with the Massachusetts Prescription Monitoring Program (PMP).

No suspension/exclusion from participation in federal health care programs.

X‑Waiver preferred.

Education: Master’s degree or higher.
