Registered Nurse- HBPC Tilton/Seacoast
U.S. Department of Veterans Affairs – full‑time (40 hrs/week) position at Manchester VA location. Eligible pay range: $76,080.00 – $135,319.00 per year. 50 days paid time off per year (26 days annual leave, 13 sick days, 11 federal holidays). Retirement: 5‑year vesting federal pension and 401(k) with up to 5% VA contribution. Health insurance via VA.
Summary Registered Nurse–HBPC in home‑based primary care delivering comprehensive, patient‑centered care to Veterans in their homes. Responsibilities include assessment, planning, implementation, and evaluation of care, medication administration, home visits, interdisciplinary coordination, and patient education.
Qualifications
English language proficiency required.
Graduated from a U.S. nursing school accredited by ACEN or CCNE, or equivalent foreign credential with full, active, unrestricted registration.
State license: active RN license, or if pursuing licensure during a master’s bridge program eligible for temporary appointment.
Preferred experience in primary or home care; 25–40 veteran patient panel.
Basic nursing education: ADN/Diploma, BSN, MSN, or doctoral degree, with corresponding experience levels as detailed in the VA qualification standard.
Physical ability to perform required duties.
Complete pre‑placement examination.
Duties
Assess, plan, implement, and evaluate age‑specific care plans.
Administer medications and procedures per VA policy.
Deliver technically competent home care (health promotion, prevention, geriatric, primary, palliative).
Maximize health capabilities, reduce unnecessary acute care visits.
Conduct home visits and manage patient inquiries (electronic, telephone).
Use PACT Almanac and other data sources for patient management.
Care plan development, monitoring, and evaluation; goal setting and treatment plans within 30 days.
Manage a panel of 25‑40 veterans; coordinate with HBPC and primary care providers.
Work Schedule Monday‑Friday, 7:30 am‑4:00 pm (Full time). Telework and virtual work not authorized. Compressed/flexible schedule not authorized.
