Nurse Practitioner – Longevity Health

About the Role We are looking for a Nurse Practitioner or Physician Assistant with a passion for working with seniors in long‑term care settings. In this role, you will be responsible for a panel of enrolled members receiving nursing home care. You will be supported by a dedicated clinical leadership team that provides education and guidance on best practices for effectively caring for our members.
You will conduct comprehensive assessments, develop individualized care plans, participate in nursing facility care team meetings, maintain frequent communication with members’ families, and ensure the care team stays aligned with members’ goals and needs.
Key Tasks and Responsibilities
Provide collaborative primary care and services to a panel of Longevity Health members, treating in the most appropriate setting based on the member’s goals of care.
Execute and deliver contracted plan’s Plan Model of Care activities, including advanced care planning and the treat‑in‑place model.
Facilitate and administer health‑plan clinical operations functions such as prior authorization, Part A and B services, medication reviews, HEDIS and STAR gap closure, and membership growth promotion.
Serve as a trusted advisor to members, loved ones, nursing home staff and facilitate member advocacy, educating care teams on member needs and early identification of changes in condition.
Deliver effective and frequent communication and coordination with members’ PCPs, loved ones, and care team members on care needs, goals of care, and treatment plans.
Support overall market goals and balance member caseload demands with team support needs.
Promote Longevity Health with members, loved ones, nursing facilities and other stakeholders.
Function autonomously and be accountable for practice; establish meaningful relationships in the nursing‑home environment.
Maintain a member‑centric, customer‑service orientation that ensures high‑quality care for Longevity Health members.
Perform other related duties as assigned.
Supervisory Responsibilities There are no supervisory responsibilities for this position.
Credentials & Coverage
State‑unrestricted licensed Advanced Practice Nurse with National Board certification (AANP, ANCC, or AACN) as Adult, Family, Geriatric or Primary Care NP; or licensed Physician Assistant NCCPA board certified with nursing home/geriatric or hospice/palliative experience.
Active DEA license registered in the state of practice.
Active national provider number (NPI) and Medicare provider number.
Eligibility to participate in Medicare as a provider and being registered.
Valid state driver’s license with good driving record and proof of automobile insurance.
Evidence of auto insurance coverage as required by your state of residence. New employees must provide proof prior to hire and on an ongoing basis and must be willing to drive up to one hour to and from work daily.
Must be present in SNF communities/buildings during business hours daily.
Education and Training
Master of Science in Nursing as a Nurse Practitioner from a nationally accredited program OR Master’s Degree from a nationally accredited program.
Knowledge and Experience
Minimum six months experience as a Nurse Practitioner/Physician Assistant with experience with geriatric/hospice or palliative care population prior to becoming an NP/PA OR a licensed new graduate NP/PA supervised by an experienced NP/PA in the LHP New Graduate Fellowship program for six months and who successfully completes all competencies for independent practice as an LHP APP.
Experience working in the nursing home environment is strongly desired.
Computer and technology proficiency required.
Excellent communication skills with physicians, patients, and family members.
Self‑directed yet works well in a team environment.
Excellent clinical skills—assessment, diagnosis, treatment, prescribing for the chronically ill population.
Independent practice with critical thinking, organization, and prioritization skills.
