Title: Nurse Practitioner – Geriatric Primary Care
Summary:
An innovative and mission-driven healthcare organization is seeking a
Nurse Practitioner (NP)
to join its
brand-new PACE (Program of All-Inclusive Care for the Elderly)
center in
Kokomo, IN . This role is perfect for an NP with a passion for geriatric care, team collaboration, and building new care models from the ground up. The center supports seniors who wish to
age in place , offering coordinated medical and supportive services in a vibrant, community-based setting.
Day to Day
Serve as a core member of the
interdisciplinary team
(IDT), working closely with social workers, PT/OT, dietitians, and other care professionals
Conduct
comprehensive health histories and physicals
for new participants
Manage and update individualized care plans, providing ongoing primary care
Provide
preventive care , immunizations, screenings, and medication management
Make
referrals
to specialists as needed and coordinate external care
Deliver care across
multiple settings : in‑center, in participants’ homes, and in contracted facilities (nursing homes, hospitals, etc.)
Collaborate
with the Medical Director and other providers to ensure high‑quality outcomes
Support the launch of the new center by helping
develop workflows, processes, and team structure
Travel up to
50%
locally as the participant census grows
Qualifications Must‑Haves
Graduate of an accredited Nurse Practitioner program
Certified as a
Family, Adult, or Geriatric NP
(AANP or ANCC)
Active
NP license in Indiana
Current
DEA registration
and
BLS certification
(AHA or Red Cross)
2+ years of NP experience , including
1+ year working with frail/elderly populations
in primary care or long‑term care
Strong understanding of
value‑based care
and
Medicare/Medicaid
models
Experience with
team‑based care
and collaborative care planning
Excitement for
building and growing a new care center
Nice to Have
Leadership or program development experience
Knowledge of PACE or similar elder care programs
Comfort with EMRs and tech tools for mobile documentation
Competitive
salary: $125,000–$150,000
(based on experience)
