Setting:
PACE (Program of All-Inclusive Care for the Elderly)
Responsibilities: Provide primary medical management in collaboration with PACE Physicians Perform assessments and develop care plans for participants Evaluate and treat participant physical complaints Provide education to participants and caregivers Function as part of the Interdisciplinary Team (IDT) Provide oversight of the PACE Clinic Perform rounds in contracted nursing homes Document participant conditions and update medical records
On-Call/Weekend Requirements: On-call every
third week
(Monday Friday, 5:00 PM to 8:30 AM; Saturday and Sunday all day)

Active
PA license
Required ANCC/AANP Board Certified
Required BLS Certification
Required CDS Certification & DEA Registration
Required 5-7 years of NP experience, preferably in
long-term care , with at least
1 year working with the frail/elderly population
Required Clean/Clear malpractice history
