St. Patrick Center, a ministry of Catholic Charities, seeks a dedicated Case Manager to join our team.
As the Case Manager, you will work collaboratively with human service agencies to ensure that services and essential resources are provided in a timely manner. You will support unhoused clients, facilitate the move‑in process, coordinate appointments, and establish financial habits to support long‑term goals. Core working hours are Monday through Friday, 8:00 am to 4:30 pm.
Overview And Responsibilities
Work directly with hospital social workers to engage, accept, and process referrals to the program.
Conduct comprehensive, ongoing assessments of clients’ mental, physical, and housing status.
Facilitate the move‑in process, including housing applications, rent payment, move‑in kits, and actual move‑in within 30 days of referral.
Provide on‑call crisis intervention in coordination with other H2HH team members, covering nighttime hours, holidays, and weekends.
Manage primary caseload.
Facilitate person‑centered, strengths‑based case management services in the office, hospital, and community settings.
Refer and assist with warm handoffs to appropriate community agencies, consistent with evolving service planning.
Coordinate and help clients with appointments and other tasks essential to medical necessity.
Act as advocate and liaison, consulting with community agencies to maintain coordination of care and transitions.
Collaborate with other H2HH team members during daily and weekly staff meetings.
Maintain client tracking system and files, documenting contacts and progress within 24 hours.
Ensure compliance with agency policies regarding work hours, building security, and personnel standards.
Perform within St. Patrick Center’s adopted care models: Housing First, Trauma‑Informed Services, and Recovery‑Oriented.
Other duties as assigned.
Knowledge And Experience Requirements
Bachelor’s degree in social work, counseling, or a related field, or an associate degree with 2–4 years of experience.
Peer Support Certification preferred.
Driver’s license required (MO residents require Class E).
Reliable vehicle with valid and current auto insurance.
Knowledge of community agencies.
Knowledge of the Housing First model.
Knowledge of Trauma‑Informed Care.
Knowledge of federal, state, and agency HIPAA policies.
Skills And Competencies Required
Understanding of mental illness and substance use.
Understanding of strengths‑based case management/wrap‑around services.
Excellent verbal and written communication skills.
Crisis intervention expertise.
Self‑motivation due to the autonomous nature of the position.
Respect for clients and staff.
Sensitivity to cultural diversity.
Ability to work in a team atmosphere.
Ability to work in a high‑pace organization.
Moderate computer skills.
