Hourly pay range:
$22.98 – $27.00
Supplemental pay:
Overtime
Mileage reimbursement:
$0.625 per mile

Rewarding Work You Will Do The Palliative Care Navigator manages scheduling for NP providers, assists with patient care coordination, and oversees administrative tasks for the Palliative Team.
Responsibilities
Collaborate with team members in the scheduling of new admissions, attending and face‑to‑face visits.
Manage all incoming processes for palliative admissions and discharges, collaborating with Hospice Teams to ensure effective transfers when appropriate.
Assist in coordination of care with palliative team members, ensuring timely communication for quality patient outcomes.
Coordinate care with physicians, home health providers, hospital case managers and other providers in the community.
Regularly communicate with the palliative team and contribute to the development of care plans based on telephonic feedback and patient status.
Coordinate with the palliative team to address resources and community care based on identified needs.
Handle incoming palliative care phone calls, providing assistance and empathy, respecting confidentiality and privacy, and communicating courteously and respectfully.
Document all patient care activity in EMR.
Promote positive public relations with patients, family members and community providers.
Follow up as appropriate with Director of Palliative Services regarding reported complaints, problems and concerns.
Participate in community events and educational conferences.
Develop and maintain a palliative care data system to track patients and outcomes.
Complete audits for the Palliative and Hospice departments as needed for patient records, documentation and billing practices.
Perform other duties and responsibilities as assigned.
Position Qualifications
Minimum education: Bachelor’s degree preferred; degree in behavioral science, nursing, public health or similar field desired.
Minimum experience: 2‑5 years working in a clinical setting.
Physical Requirements
Ability to lift or carry a minimum of 30 lbs.
