Summary
Position Type : Non‑exempt, full‑time or part‑time, hourly role.
Scope of Role : Provide direct support within a single program or unit; typically support 4–6 individuals in residential settings or up to 10 individuals in periodic programs.
Key Responsibilities :
Deliver support as outlined in individual service or program plans—socialization, health maintenance, medication administration, skill/behavioral development.
Assist with daily activities: personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.
Essential Job Functions
Direct Support : Assist with socialization, behavioral development, personal care, housekeeping, recreation, transportation, community orientation, shopping, financial management, citizenship, and other daily living activities.
Records : Accurately document progress and activity; review records and logs; maintain confidentiality.
Asset Management : Make cost‑effective purchases within budget and based on individual preferences.
Relationships : Maintain professional relationships with individuals, families, guardians, case managers, and visitors; implement Sevita Customer Service Standards.
Reporting : Report abuse, neglect, medical incidents, behavioral incidents, and other events in accordance with Sevita policy and external requirements.
Rights & Privacy : Respect and protect the rights of individuals served; practice universal precautions; assist individuals in exercising their rights.
Health Care Responsibilities
Appointments : Accompany individuals to medical appointments; relay orders and information to providers.
Medications : Administer or support self‑administration of medication; document delivery; report errors; secure controlled substances.
General Health Care : Monitor health, document concerns, communicate with nurse or supervisor, call 911 in emergencies, follow care directives.
Medical Supplies & Equipment : Follow procedures, report malfunctions.
Meal Preparation : Assist in food preparation per menu and individual choice; handle special diets; monitor mealtimes.
Employment Responsibilities
Training & Attendance : Attend orientation, ongoing training, and monthly staff meetings.
Scheduling : Work scheduled hours; arrive on time; stay until replacement arrives; notify supervisor when unable to work.
Teamwork & Safety : Work collaboratively, show respect, communicate openly, maintain a safe workplace, and report hazards.
Maintenance & Safety
Vehicles : Transport individuals safely, follow laws, use safety equipment, report accidents.
Housekeeping : Perform daily and seasonal housekeeping; maintain cleanliness and safety; report hazards.
Repair & Safety
Perform minor repairs; maintain walkways; monitor water temperature for bathing; participate in safety drills.
Qualifications
Education & Experience :
High school diploma or equivalent preferred.
Six months of experience in human services preferred.
Must be 18 years of age.
Working knowledge of computers.
Certificates, Licenses & Registrations :
Valid driver’s license in good standing.
Car registration and insurance if providing transportation.
All state‑required trainings and certifications completed within mandated timeframes.
Drug & Background Testing : Must submit to drug testing prior to employment and randomly thereafter; must pass criminal background checks and relevant registries.
Physical Requirements : Must be able to exert up to 100 pounds of force occasionally, up to 50 pounds of force frequently, or up to 20 pounds constantly to move objects.
