As our Clinical Dietitian, you will assess the nutritional status of our patients using recognized assessment tools and recommend and counsel patients, family, and caregivers on the appropriate nutrition plan to improve the patient's health and well-being.
Every day, you will work with the general patient population and patients in more complex levels of care under the supervision of a Clinical Dietitian Intermediate or Clinical Dietitian Lead to perform nutritional assessments using age and gender-appropriate standards and cultural, religious, and ethnic backgrounds. You will conduct routine screenings to identify patients at nutritional risk and collaborate within an interdisciplinary environment to contribute to facilitating care for those needing a higher level of care, all while demonstrating excellent customer service to patients, families, and caregivers. Responsibilities
Develop an intervention plan which includes appropriate feeding route, mode, and nutrition prescription unique to patient's specific needs, in accordance with physicians' orders.
Monitor and evaluate patient response to interventions by meeting specific measurable goals documented in the nutrition care plan in a timely and thorough manner in the medical record.
Update plan of care as necessary and follow up with the patient as needed.
Collaborate within an interdisciplinary environment to facilitate care and identify patients at nutrition risk or needing higher level of care.
Collaborate with the Food and Nutrition Services team to enhance patient experience.
Minimum Job Requirements
Bachelor's Degree with completion of accredited dietitian program (ACEND – Accreditation Council for Education in Nutrition Dietetics)
Valid registration as a Registered Dietitian with the Commission on Dietetic Registration (CDR) within 3 months
