Job Overview We’re seeking a dedicated Behavioral Health Consultant to support providers as a key member of an Integrated Behavioral Health team. This role focuses on delivering timely, brief interventions, supporting patients with emotional and health-related challenges, and coordinating referrals to internal and external behavioral health services. The ideal candidate is patient-centered, resourceful, and committed to increasing access to behavioral health care in a primary care setting.
Responsibilities
Partner with the primary care team to support patients with behavioral and emotional health needs
Provide care management interventions, behavioral health education, and short-term solution-focused support
Coordinate referrals and link patients and families to community resources
Maintain accurate documentation in accordance with program guidelines
Participate in staff training, program evaluation, and quality improvement initiatives
Uphold confidentiality and organizational policies
