Position Summary We are seeking a skilled
Mental Health Therapist
to join our integrated primary care team. The therapist will provide short-term (10–12 sessions), solution-focused therapy to adults with mild to moderate mental health concerns. This role collaborates closely with medical providers to support coordinated, whole-person care.
Key Responsibilities
Conduct assessments and provide evidence-based therapy such as CBT, behavioral activation, and problem-solving therapy.
Collaborate with primary care teams to develop and implement treatment plans.
Screen adults and older adults for behavioral health needs, trauma, and substance use.
Facilitate psychoeducational groups and make appropriate referrals.
Maintain timely, accurate, and compliant clinical documentation.
Participate in interdisciplinary meetings and care coordination activities.
Qualifications
Master’s in Social Work, Marriage & Family Therapy, or Doctorate in Clinical Psychology.
Current California licensure as an
LCSW
or
Psychologist , or registration as
ASW/AMFT
(supervision provided).
Minimum one year of clinical experience.
Familiarity with DSM-5 and evidence-based treatment approaches.
Basic proficiency in Microsoft Office and EMR systems (OCHIN Epic preferred).
Preferred Qualifications
Background in integrated or community health care.
Training in trauma-informed care.
Spanish language fluency is a plus.
