Licensed Practical Nurse (LPN)
– Shorewood Behavioral Health
Location: Madison, WI – a state‑of‑the‑art, 120‑bed behavioral health facility that provides comprehensive acute and outpatient psychiatric care for adolescents, adults, and seniors. Programs include partial hospitalization (PHP), intensive outpatient (IOP), electroconvulsive therapy (ECT), and other specialty services tailored to community needs.
Shift Schedule Days: 7:00 am – 7:00 pm (full‑time) Nights: 7:00 pm – 7:00 am (full‑time)
Purpose Statement Provide nursing care and services within the standards and scope of nursing practice as defined by state Nurse Practice Act regulations, facility and corporate policies, and protocols. Demonstrate a positive, empathetic, and professional attitude toward customers at all times, with patient safety as a top priority.
Essential Functions
Monitor patient health and administer basic care using the nursing process within the LPN/LVN scope of practice to achieve care plan objectives.
Serve as a role model for customer service and exhibit a sense of urgency related to patient safety.
Collaborate with team members to ensure patients’ physical, biopsychosocial, developmental, and cultural needs are met; address and resolve complaints when needs are unmet.
Provide basic comfort for patients and implement individualized nursing interventions.
Apply appropriate interventions to progress patients toward wellness and maintain wellness.
Discuss care provided with patients and listen to their concerns; report status changes and concerns to registered nurses and physicians.
Communicate therapeutically and professionally with patients, visitors, coworkers, and other treatment collaborators.
Ensure safe medication delivery in accordance with policies, procedures, physician orders, and current practice.
Maintain professional nursing standards in line with facility policy, external governing bodies, credentialing, and psychiatric nursing standards.
Responsible for thorough, accurate reporting and documentation of patient symptoms, responses, and progress.
Other Functions
Perform additional duties as assigned.
Education, Experience, and Skill Requirements
Graduation from an accredited nursing school.
Preferred: One or more years of nursing experience in a clinical setting similar to services offered.
Licenses, Designations, and Certifications
Current LPN/LVN license as required by state.
CPR and de‑escalation/restraint certification required (training available upon hire).
First aid certification may be required based on state or facility policies.
