Our client is seeking a Licensed Practical Nurse (LPN) to provide telehealth services. This fully remote position works with clinical staff to provide patient education, monitor patient conditions, and ensure condition changes are reported to the appropriate treating provider. The role works directly with patients in a telehealth environment and communicates clinical concerns to providers.
Key Responsibilities
Engage patients in individualized, one-on-one sessions to reinforce care plans
Deliver patient-centered nursing support
Gather and update clinical information during virtual visits (health history, medications, vital signs from home devices, patient-reported symptoms)
Guide patients in applying care plans with practical strategies for healthier routines (diet adjustments, activity tracking, stress management)
Track patient progress through scheduled follow-ups and review of monitoring data, ensuring timely communication of concerns or changes to the clinical team
Provide clear education on medication schedules, recognition of early warning signs, and preventive care practices, empowering patients to manage their conditions confidently
Qualifications
Active state LPN license (or multi-state license) is preferred
3+ years of clinical experience as a Licensed Practical Nurse
Experience with cardiac care and heart failure is preferred
Location: Texas, United States. Salary: $52,000.00–$62,000.00 per year.
