The role includes floating to Worthington, MN two days per week.

Job Description:

Motivated and compassionate PT required to deliver high-quality patient care to older adults in a collaborative rehab environment.

Responsibilities:

Evaluate, plan, and deliver physical therapy treatments to patients in SNF and sub‑acute rehab settings.

Create individualized treatment plans to help residents improve mobility, strength, and functional independence.

Accurately document all therapy sessions, progress notes, and discharge plans per facility and regulatory requirements.

Work closely with occupational therapists, speech‑language pathologists, nursing staff, and physicians to coordinate patient care.

Maintain an 85% productivity standard.

Travel between Lamberton and Worthington facilities as scheduled.

Qualifications:

Current and valid PT license in Minnesota (or eligibility for MN licensure).

Graduate of an accredited Physical Therapy program.

Experience in a Skilled Nursing Facility (SNF) or geriatric rehab preferred.

Excellent communication, organization, and time‑management skills.

Reliable transportation for float days.

Position Highlights:

Contract Length: Flexible or renewable assignment.

Hours: 37 guaranteed weekly hours (GWR).

Setting: Skilled Nursing Facility (SNF).

Locations: Primary – Lamberton; Float – Worthington (2 days/week).

Schedule: Monday–Friday, day shift.
