Shift: Days, Monday–Friday (some evenings and weekends as required)
Physical Requirements: Extended periods of standing and/or sitting; traveling long distances by car
Job Summary The College of Nursing (CON) has contracted with the Cabinet for Health and Family Services, Department for Medicaid Services (DMS) to create a team of licensed Registered Nurses that will perform assessments utilizing the Home and Community Based (HCB) waiver assessment tool for the frail and elderly population and physically disabled. The role will identify, develop and present information to educating professional staff, families and individuals seeking HCB Services while also making sound independent determinations as to the level of care and services needed for this vulnerable population. The majority of in‑home assessments will be based in Morgan County, but the candidate may be asked to complete assessments in surrounding counties. Mileage reimbursement will be provided and flexibility granted for scheduling in‑home assessments to meet both personal and client needs.
Skills / Knowledge / Abilities
Knowledge of physical and psychological development
Excellent analytical and decision‑making skills
Effective communication and interaction with physically disabled and elderly individuals
Ability to travel throughout the state on a regular basis with occasional overnight out‑of‑state trips
Preferred Education / Experience
Experience working with the elderly and physically disabled
Driver’s license (highly desired)
