Job Summary AMB: Diabetes Treatment Center – Full‑Time, Day Shift. The Patient Navigator‑Wound/Ostomy Care outpatient navigator operates under the supervision of licensed professionals (RN, NP, MD) in a hospital‑based outpatient clinic. The role coordinates patient referral, collects and reviews medical records, verifies insurance eligibility, and facilitates inpatient and outpatient wound, ostomy, and continence care for patients with acute and chronic conditions at a large tertiary academic medical center with level‑one trauma and Magnet® recognition. Responsibilities include pre‑surgery work‑up, post‑surgery healing, gastrointestinal evaluations, hyperbaric oxygen treatments, wound debridement, dressing and compression therapy, and patient and caregiver education. The navigator ensures timely appointments, supports clinic operations, schedules and registers patients, manages billing and charge capture, and assists department leadership in delivering patient‑centered, efficient, and cost‑effective care.

Responsibilities and Scope of Practice The Navigator functions in a variety of roles, including patient care, scheduling and registration, billing and charge capture, and secretarial duties. The position performs designated procedures and treatments that require specialized training for unlicensed assistive personnel, while remaining within the scope of practice for unlicensed assistants. Key duties include:

Coordinating patient referrals and obtaining necessary new patient information before admission.

Retrieving required medical record information from referring physicians or facilities.

Exploring insurance eligibility and obtaining authorizations prior to initial clinic visits.

Ensuring patient appointments are sequenced appropriately based on diagnosis and stage of care.

Assisting patients to navigate Loma Linda University Health services for timely treatment.

Contributing to efficient clinic operations and supporting leadership in delivering patient‑centered care.

Demonstrating professional behavior, communication, and customer‑service excellence to achieve high patient satisfaction.

Performing basic patient care activities with proper body mechanics and safety precautions, and following policies and procedures.

Education and Experience High School Diploma or GED required. Proof of completion of an approved Certified Nursing Assistant (CNA) or Medical Assistant (MA) program, or a nursing fundamentals course from a school of nursing. Minimum six months of experience as a MA or CNA in a clinic or acute care setting preferred; minimum six months as a unit or medical secretary preferred.

Knowledge, Skills and Competencies Knowledge of unlicensed assistant personnel scope of practice and capacity to perform patient care, scheduling, billing, and secretarial duties. Demonstrated competencies in patient care fundamentals, safety, and customer service. Strong communication skills in English; Spanish preferred. Proficient with Microsoft Office, Outlook, electronic medical record systems, and related software. Ability to read, write legibly, perform basic math, and manage multiple patient assignments effectively. Ability to work calmly under pressure, collaborate with licensed providers, and maintain professional behavior at all times.

Licensures and Certifications Basic Life Support (BLS) certification issued by the American Heart Association required.
