MDS Care Plan Coordinator
Arcadia Care is looking for an experienced MDS Care Plan Coordinator to ensure timely and accurate resident assessments and interdisciplinary care plans that meet Federal and State guidelines. The role involves identifying resident acuity, determining care needs, and communicating expectations to families and responsible parties.
Base pay range $75,000.00/yr - $80,000.00/yr
Essential Duties
Integrate nursing, dietary, social recreation, restorative, rehabilitation, and physician services for appropriate assessment and reimbursement.
Complete all Medicare/Medicaid case‑mix documents to assure appropriate reimbursement for care and services within the facility.
Conduct continual Minimum Data Set (MDS) reviews to assure achievement of optimal allowable Resource Utilization Group (RUG) category.
Oversee the overall process and tracking of MDS/Prospective Payment System (PPS) documentation and submission.
