Job Summary And Qualifications The Physical Therapist (PT) provides high‑quality, patient‑centered care by managing the complete process of therapeutic care for an assigned group of patients in coordination with physicians and the patient care team. The PT conducts patient assessments, develops and implements individualized care plans, and directs the majority of the patient’s therapy throughout the continuum of care.
Responsibilities
Provide evaluations and treatment interventions within the scope of practice.
Assess and evaluate therapeutic/rehabilitative/functional status and participate in developing the total plan of care.
Deliver purposeful and skilled therapeutic care using procedures and techniques to achieve physician’s plan of care objectives.
Instruct patient/family/caregiver on therapies, exercises, appliances, or prosthetic devices.
Prioritize caseload, determine daily staffing needs and coordinate assignments/handoffs.
Qualifications
Graduate of an APTA‑accredited school with a BS, MS, or Doctorate in Physical Therapy.
Active Physical Therapy license in the State of Colorado.
Current certification in Basic Life Support for HealthCare Providers (BLS). Must obtain within 30 days of employment start date.
Successful completion of at least one clinical rotation or work experience in acute care hospital, long‑term acute care facility, skilled nursing facility, or acute rehabilitation setting.
Licensed Physical Therapist (LPT).
Two years of experience preferred.
Additional Information No travel required.
