Overview At Rehab Without Walls, we specialize in providing high-quality care to individuals with neurological conditions in the comfort of their own homes and community settings. We’re looking for a dynamic and empathetic Licensed Clinical Social Worker (LCSW) / Licensed Social Worker (LSW) to join our team and make a profound impact on patients’ lives outside the traditional clinic setting.
Responsibilities
Conduct comprehensive evaluations, assess the social, emotional and support needs of patients and their families, and set measurable objectives and goals in conjunction with the rehabilitation team. Adjust treatment plans to achieve maximum independence and/or function.
Identify and assist patients and/or families in utilizing appropriate community resources to achieve measurable objectives. Use special skills and judgment to resolve adverse social and emotional responses related to the injury/illness.
Provide direct social work intervention and instruct and counsel patients and family members in coping with social and emotional responses related to the injury/illness.
Regularly reassess the effectiveness of treatment plans, patient progress toward goals, and the need to modify goals or treatment.
Communicate patient needs and progress to the treatment team, physicians, patients, families, and others as applicable.
Consult with medical providers regarding patient evaluation & treatment, progress, and discharge from treatment.
Observe, document, and report changes in the condition/status of the person receiving services to the Clinical Coordinator, Program Manager, DCM, attending physician, and other treatment team members.
Maintain and submit evaluation, treatment, and billing documentation in the required formats and timelines stipulated by the company, facility, funding source, and local, state, and federal regulations.
Adhere to the company's Code of Business Conduct, policies, HIPAA, and corporate compliance programs, as well as applicable professional practice standards and state/federal laws.
Perform other duties as assigned.
Qualifications
Master's Degree in Social Work or Marriage and Family Therapy (LCSW, MSW, MFCC, or LPC)
Current social work or marriage and family therapist licensure/certification in the state(s) of practice required by individual states.
Minimum of one year of experience in a rehabilitation setting serving individuals with traumatic brain injury, spinal cord injury, or other acute diagnoses.
Current BLS (Basic Life Support), First Aid, and CPR certifications.
Effective and professional interpersonal, verbal, and written communication skills.
Ability to work independently and as part of a team.
Strong organizational skills and attention to detail.
