Responsibilities Verify the Plan of Care and provide care according to RN delegation and physician orders. Document patient care goals and outcomes in skilled nursing notes. Communicate significant findings to RN Case Manager, physician and other team members. Report unsafe conditions to the appropriate RN Case Manager and physician. Assist patients in learning self‑care techniques. Implement all available actions to prevent avoidable hospitalizations and ER visits.
Qualifications Graduate of an accredited Certificate, Diploma, or Associate School of Nursing. Current State License as an LPN. 1 year of experience in a clinical care setting. Experience in a hospice or home health environment preferred. 50% travel required. Ability to sit, stand, bend, lift and move intermittently and lift 50‑100 lbs. Valid driver’s license and reliable transportation.
Report To Clinical Team Manager.
