Job Summary Provide day‑to‑day support and services for participating veteran households. Requires experience in homelessness prevention, rapid re‑housing, and transitional supportive services. Act as a key liaison from intake and assessment through attainment or maintenance of permanent housing, promoting accessibility, outreach to targeted populations, and providing case management, advocacy, and wrap‑around supports with the VA and community partners.

Responsibilities

Coordinate outreach efforts for the SSVF Program, including establishing an outreach plan to target the three categories of participants, scheduling visits to the VA, shelters, and other high‑traffic locations, and maintaining communication with the VA and key housing providers.

Screen potential program participants for eligibility using VA‑approved forms and verify veteran and income status with applicable agencies.

Conduct housing and comprehensive supportive services assessments and develop a Family Services Plan.

Provide service coordination between the program and community partners for participants and their families/supporters.

Collaborate with participants, family members, housing agencies, and professionals to revise service plans and develop placement, stabilization, and follow‑up plans.

Arrange for housing support and wrap‑around services, connecting participants to all eligible VA and mainstream resources to support transition or maintenance of permanent housing.

Travel to housing sites to support participants, monitor progress, and address health, behavioral, or life‑skills issues.

Communicate effectively (under confidentiality rules) with clients and service teams to rapidly address and correct issues related to living skills, employment, education, family, and socialization.

Ensure appropriate use of and documentation of any temporary financial assistance.

Support veterans and families in rapid re‑housing, promoting financial independence and community relationships.

Ensure participant category reassessments are completed according to SSVF requirements.

Participate in program supervision, attend required staff and Project Management Team meetings and trainings, and contribute to team‑based collaborative planning.

Complete all correct reporting in HMIS and other required systems.

Complete other duties as needed and requested.

Effect on End Result

Rapidly addresses veteran housing, employment, family, social support, and life‑skill issues, adjusting services as needed to maintain housing, economic stability, and community integration.

Ensures veterans receive appropriate transition and service plans.

Maintains complete and timely client file documentation and statistical reporting.

Qualifications

Bachelor’s degree in counseling psychology, rehabilitative counseling, or related human services field.

Other Requirements

Driving requirement: personal vehicle required for company business; must meet Agency MVR guidelines.

Valid driver’s license in jurisdiction of residence.

Vehicle liability coverage if driving personal vehicle (may be required to transport clients).

May be required to drive a van.

Acceptable criminal background check and OIG exclusion check.

Negative drug screening.

CPR/First Aid certification maintained during employment.

Physical Requirements

Ability to safely operate a motor vehicle to transport self, consumers, and program supplies.

Physical ability to travel to assigned locations and perform basic and light home maintenance activities (stand, stoop, bend, reach, pull, push, lift, grasp, climb, talk, see, hear).

Operating office equipment that requires continuous or repetitive hand/arm movements.

Ability to remain seated for extended periods.
