Position Summary We’re seeking an experienced Home Health Physical Therapist to deliver high‑quality, patient‑centered care in the home setting. This role offers autonomy, flexibility, and strong interdisciplinary support.

Responsibilities

Perform comprehensive patient assessments

Develop and implement individualized care plans

Provide skilled PT to improve mobility, strength, and function

Educate patients/caregivers on exercises and safety

Document visits accurately in EMR

Collaborate with physicians and care team

Ensure compliance with Medicare & HIPAA standards

Qualifications

Graduate of accredited PT program

Active Virginia PT license

Minimum 3 years clinical experience (home health preferred)

OASIS experience preferred

EMR experience (WellSky/Kinnser/HCHB a plus)

Ability to supervise PTAs

Reliable transportation & valid driver’s license
