Physical Therapist (PT) - Spotsylvania County Human Touch Home Health is seeking an experienced Physical Therapist to provide in-home therapy services in Spotsylvania County.

Responsibilities

Perform in-home PT evaluations

Develop and implement treatment plans per physician orders

Provide skilled therapy to improve mobility, strength, and balance

Educate patients and caregivers

Complete timely documentation and collaborate with care teams

Supervise PTA as applicable

Qualifications

Licensed Physical Therapist – Virginia

Minimum 3 Years PT Experience (home health preferred)

EMR experience (WellSky/Kinnser or Homecare Homebase preferred)

OASIS experience a plus

Valid driver’s license & reliable transportation
