Job Summary As a Case Manager in our Rehabilitation program, you’ll guide patients and families through the healing process, ensuring each patient receives high-quality, personalized care that supports recovery, independence and hope. You will coordinate and direct rehabilitative care, collaborate with program directors and the interdisciplinary team, and promote HCA Healthcare’s values of compassion, respect and excellence.

Responsibilities

Guide patients and families through program orientation, explaining the rehabilitation philosophy, Medicare and insurance benefits, discharge criteria and patient rights.

Coordinate education for patients, families and caregivers to encourage participation in treatment planning, goal discussions and family conferences.

Develop and implement individualized treatment plans that reflect the patient’s strengths, needs and personal recovery goals.

Complete psychosocial assessments and discharge planning evaluations, clearly documenting findings and communicating needs across the care team.

Collaborate closely with the Rehab Program Director, Facility Case Management Director and interdisciplinary team to ensure seamless, patient-centered care.

Promote HCA Healthcare’s values of compassion, respect and excellence through every patient and family interaction.

Qualifications

RN with current state licensure; BSN preferred.

Three years of clinical, hospital nursing experience required (preference for those with at least 2 years of case management).

Experience in critical care or at least 2 years of critical care experience.

Certification in case management or utilization review preferred.

InterQual experience preferred.

Location & About the Facility HCA Florida St. Lucie Hospital is a 229-bed acute care facility accredited by the Joint Commission. Since 1983, the hospital has provided quality care to the Treasure Coast community, offering a full range of services including birthing, cardiac care, cancer care and surgical specialties. It is a Certified Primary Stroke Center and an American College of Cardiology accredited Chest Pain Center.
