Licensed Practical Nurse (LPN) – Lafayette Manor

Responsibilities
Provide and direct resident care in accordance with SNF/ICF guidelines, State and Federal Rules & Regulations for Nursing Homes, Resident Bill of Rights, Nursing Policies, and the organization’s Personnel Handbook.
Observe, assess, and report changes in resident’s condition to immediate supervisor, attending physician, family or designated contact.
Call or accept verbal orders from resident’s physician, transcribe them according to established procedures.
Move medication cart, administer drugs, transfer treatment supplies, document appropriately, and report any drug or treatment errors.
Administer drugs under supervision of a Registered Nurse; pass oral medications and give intramuscular and subcutaneous injections per policy.
Adjust drip rates, change IV tubing, cleanse and check IV sites, and discontinue IV fluids as needed.
Perform treatments and assist in procedures such as insertion of retention catheter, tube feedings, dressing changes, suctioning, and specimen collection.
Initiate and update total plans of care and problem lists in resident’s chart and MAR/TAR at all times.
Participate in multi-disciplinary staffing meetings, supervise staff, and support professional development.
Maintain confidentiality, uphold nursing standards, and adhere to the Professional Appearance Standards and Dress Code Policy.
Provide life‑saving care (CPR, Heimlich, etc.) and assist physicians during examinations or treatments.
Contribute to policy development, training, and continuous education to enhance nursing care programs.
Qualifications
Licensed Practical Nurse currently licensed by the State of Wisconsin.
Knowledge of nursing and medical practices, including laws, regulations, and guidelines for nursing home operation.
Basic leadership and supervisory skills.
Physical ability to lift and carry light objects, transfer residents up to 75 lbs. with assistance, and push/pull medication carts (10–30 lbs.).
Physically able to reposition residents, manipulate carts through narrow areas, stand or walk up to 75% of an eight‑hour shift, and perform tasks requiring stoop, bend, squatting, and manual steadiness.
Good hand/eye coordination for blood pressure, temperature measurements, injections, and flow rate adjustments.
Ability to wear gloves and protective equipment as required.
