PDN RN/LVN: 11 y/o (GB) Fort Worth, TX | Sun/Sat (10A-10P) Mon-Fri (7P-7A) VIVA Pediatrics is a home health services company with a specialty in private duty nursing, therapy and skilled nursing visits for infants and children. We are committed to providing each patient with an exceptional level of care.
Assess home health patients to identify the physical, psycho-social, and environmental needs and document findings.
Implement, develop, and document the plan of care to ensure quality and continuity of care.
Provide care utilizing infection control measures that protect staff and patients (OSHA); compliance with state, federal, and local regulations.
Comply with all VIVA Pediatrics policies and procedures and promote the Agency philosophy.
Communicate effectively with patients, families, team members, and other health professionals through clinical notes, case conferences, and evaluations.
Notify supervising nurse immediately of changes in the client’s condition.
Provide health education to client and/or caregiver.
Document interim changes of plan of care in a timely fashion.
Requirements
At least 1 year of experience or 6 months pediatric home health as a licensed RN or LVN.
CPR Certification.
Valid driver’s license.
