The Licensed Integrated Mental Health Therapist (LMHT) is responsible for independently providing assessments, treatment planning, and psychosocial therapy within a primary care clinic setting for a diverse population of preventative, acute care, and chronic disease management patients. The incumbent uses a variety of theoretical treatment models and interventions, documents treatment in keeping with department standards, and stays current on community resources to facilitate linkage and advocacy. FTE: 1.0, Shift: Day, Schedule: Day. Responsibilities
Assesses patient symptoms and creates customized treatment plans based on individual needs. Screens patients for common behavioral health disorders using PHQ‑9, GAD‑7, Columbia Suicide Risk Assessment, or other tools as required. Monitors patients for changes in clinical symptoms and treatment side effects, aiming to reduce symptoms. Supports psychotropic medication management prescribed by PCPs, focusing on adherence, side effects, and effectiveness. Tracks patient follow-up and clinical outcomes using a registry, identifying and re‑engaging patients as needed. Facilitates care plan changes for patients not improving, including medication or psychosocial adjustments or referrals for additional services. Manages behavioral health crisis as they arise in a primary care setting. Provides flexible behavioral health services to patients on and off caseload, both through scheduled appointments and immediate interventions via warm handoffs during medical visits. Requirements
Master’s degree from an accredited institution in Social Work, Counseling, Marriage & Family Therapy, or related field. Licensed Mental Health Professional (LMFT, LMHC, or LICSW) as defined within HB 1724. Minimum two (2) years of experience in a behavioral health setting. Our Values
