Job Summary

Responsibilities
Administer medications and treatments as prescribed by physicians
Monitor and record vital signs and medical information as needed
Assist with activities of daily living, including bathing, dressing, and grooming
Communicate with patients and families regarding care and treatment
Make periodic checks to ensure the prescribed treatments are being properly administered by nursing staff
Requirements
Active LPN license in the state of Kentucky
Experience in long‑term care preferred
Excellent communication and interpersonal skills
Ability to work independently and as part of a team
Flexibility to work weekends and night shifts
Medical Specialty: Geriatrics
Physical Setting: Long term care
License/Certification: LPN (required)
Work Location: In‑person
