Working Hours: 8:00 – 4:30 (variable). Requires at least 3 home visits per week and rotating HomeHealth calls.

Role Summary Baptist Health Home Health/Hospice RN Case Manager provides, manages and coordinates care and services for an assigned caseload of patients.
Responsibilities
Coordinate and manage patient care and services.
Handle a caseload of patients requiring home health and hospice care.
Collaborate with interdisciplinary teams to create comprehensive care plans.
Ensure timely home visits and conduct rotating home health calls.
Requirements
Graduation from an approved Nursing program.
Current Arkansas RN License.
Current Arkansas Driver's License.
Minimum 1 year of nursing experience (Home Health/Hospice experience preferred).
Benefits This position will be authorized 80.00 hours bi-weekly.
