Overview Case Manager of Rehabilitation coordinates comprehensive care and discharge planning for patients in a rehabilitation setting. This role involves developing and updating the Individual Plan of Care (IPoC) in accordance with CMS guidelines, collaborating with interdisciplinary teams to address barriers to care, and maintaining clear communication with patients and caregivers. Certification in Case Management or Rehabilitation Nursing is preferred.
Responsibilities
Coordinate comprehensive care and discharge planning for rehabilitation patients.
Develop and update the Individual Plan of Care (IPoC) per CMS guidelines.
Collaborate with interdisciplinary teams to address barriers to care.
Maintain clear communication with patients and caregivers.
Participate in performance improvement initiatives and ensure timely, accurate documentation.
Facilitate family conferences to support goal achievement and discharge planning.
Qualifications
Current licensure as Registered Nurse or Social Worker, or other healthcare professional licensure (e.g., Respiratory Therapist, Physical Therapist, Speech-Language Pathologist, Occupational Therapist).
Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ARN, ACM, ANCC RN-BC).
Minimum of 2 years of social work or case management experience in an inpatient setting; acute/rehabilitation hospital experience preferred.
Schedule and Location Job Type: Full-Time
Schedule: 8:00 am – 4:00 pm at Mercy Rehabilitation Hospital Oklahoma City South and North campuses.
