About the Role Ascend Medical is seeking a warm, experienced therapist with a passion for working with older adults to join our Behavioral Health Team. This independent contractor position is ideal for a Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT) who enjoys building relationships and delivering impactful care on-site within senior living and long-term care communities. In this role, you’ll provide in-person therapy to older adults experiencing a range of emotional and behavioral health challenges—from depression, grief, and anxiety to cognitive decline, caregiver stress, and life transitions. You’ll work closely with our interdisciplinary care team to support patients where they live, helping them feel seen, heard, and empowered.
Responsibilities

th locations).Conduct psychosocial assessments and clinical diagnoses u
sing DSM-5-TR.Develop and update personalized treatment plans every 90 days or as clinica
lly indicated.Deliver evidence-based therapeutic interventions tailored to geriatri
c populations.Address mental health challenges such as anxiety, depression, grief, adjustment disorders, cognitive decline, and care
giver fatigue.Collaborate with on-site care teams, including medical providers and f
acility staff.Session documentation, consistent with best practices and insurance reimbursement standards completed in Athena (EMR) within 24 hours of ser
vice delivery.Maintain accurate, timely clinical documentation and uphold all licens
Qualifications
Active licensure in Georgia (LPC, LCSW, or LMFT); additional state li
censure is a plus.Demonstrated experience in geriatric care, ideally within senior living or skilled nur
sing environments.Experience facilitating group therapy sessions, with the ability to adapt approaches to diverse s
enior populations.Strong interpersonal skills and comfort working independently within

Pay range and Compensation Package This role will begin at 10 hours per week, split between two senior care communities: 5 hours per week at Woodstock, 5 hours per week at Acworth. As patient panels and programs expand, there will be opportunities to increase hours and grow the schedule. This is a 1099 independent contractor position. Compensation is $81 per completed patient visit. To support onboarding and initial patient scheduling, there will be a 90-day guaranteed minimum of 10 visits per week (equivalent to $810 weekly). After the 90-day onboarding period, compensation will transition to a per-visit model, with earning potential increasing as the volume grows.

Work Environment While this role is primarily in-person, some virtual follow-ups may be arranged as appropriate. As a contractor, this role does not include W-2 benefits such as health insurance or paid time off, but it offers substantial professional autonomy and work‑life flexibility.
