Reporting to the Clinical Coordinator, the ideal individual is flexible, compassionate, and professional. You will provide effective case management, clinical perspective, and service coordination for families in accordance with the Arizona Practice Model 12 Principles.
We provide primary care, nutrition, counseling and group sessions, addiction treatment, children’s services, housing, and pharmacies. Location: 3864 N 27th Ave, Phoenix, AZ 85017
Schedule: Full‑Time, Employed – Monday to Friday, 9 am–6 pm. No weekends. Additional language differential pay is available. Full benefits, including 401(k), PTO/PST, and more.
Position Summary The Children’s Case Manager provides effective case management, clinical perspective, and service coordination for families in accordance with the Arizona Practice Model 12 Principles. Under the Agency’s policies and professional requirements, the case manager participates as a member of an interdisciplinary clinical team. Responsibilities include conducting intake, coordinating treatment, managing 6‑month and annual assessments, hospital admissions and discharges, and supporting children involved with the Department of Child Safety receiving services.
Duties
Conduct the engagement process with families to identify supports as outlined in the Arizona Model, Child and Family Team (CFT) process, and Regional Behavioral Health Authority (RBHA).
Facilitate CFT/Service Plan meetings according to the Arizona Model and RBHA guidelines to create individualized service plans.
Ensure families understand the behavioral health process, the role of CFT facilitator, and link them to natural or community‑based supports.
Coordinate and schedule CFT/Service Plan meetings with natural, formal supports, and system stakeholders.
Submit appropriate referrals to clinical outpatient services within agency and other behavioral health providers identified by the CFT.
Update and maintain electronic medical records on each case, including annual assessment updates, RBHA demographics, Child and Adolescent Service Intensity Instrument (CASII), and service plans.
Complete strength assessments and crisis/safety plans with families according to RBHA and ADHS guidelines.
Develop positive supportive relationships with consumers, families, stakeholders, and other Terrros employees.
Complete Intake/Core Assessment as needed, per RBHA and ADHS guidelines.
