The Registered Nurse is responsible for the coordination of ongoing services among the patient, the patient's family, and the health care team. The Registered Nurse facilitates the planning of care for patients with wounds to promote maximum benefits from available health care services to promote continuity of care. The Registered Nurse may provide direct patient care services or supervision of others in the provision of these services. The wound care nurse will provide necessary treatment for patients suffering from wounds and/or lymphedema and assist with developing evidence-based programs. Other assigned duties. Essential Job Duties

Evaluates assigned patients gathering data from a variety of sources, such as patient records, interviews, observation, and team members. In accordance with patient's assessment and physician's prescriptions. Develops treatment plans for assigned patients including specific goals and activities. Re-evaluates and modifies plan at periodic intervals and as necessary. Discuss treatment plans and patient progress with physician and appropriate hospital staff, recommends alterations to the treatment plan when considered advantageous to therapy purpose. Exercises judgment in the delegation of activities to ensure that patient care is provided by staff with identified competencies in meeting individual patient and family needs. Acts a wound care educational resource for hospital staff and physicians. Confers with patients and families to address discharge needs i.e. regimen of home treatment, and other details of required care (referral to other facility or healthcare providers). Works to promote continuity of patient care with outpatient services, home health agencies, and other facilities. Demonstrates knowledge of principles of wound care/ ostomy care/ and lymphedema care through education for patients and family. Providing education in wound care, dressing applications, pressure ulcer care, compression therapy, diabetic foot care, venous disease, off-loading, and underlying disease processes. Facilitates departmental operations through daily completion of all required documentation, patient charges, and other documentation. Education and Experience
Required Education And Experience: Graduate of an accredited nursing program ADN or BSN Preferred
2-4 years related experience Required Licenses / Certifications
Current RN license in the state of Texas, BLS certificate, and wound care associated certification (i.e. WCC, WOCN, or CWS and HBO) within three (1-3) years of employment. Seniority level
