Service Care Coordinator Gilead Community Services is seeking an attentive and recovery-oriented Service Care Coordinator to join our multi-disciplinary residential program staff.

About The Position The Service Care Coordinator for Gilead’s

New Britain area

residential substance abuse program works

Monday-Friday 8am-12p

providing recovery oriented clinical case management services to individuals with significant histories of substance abuse and co-occurring mental health issues. The individual in this role supports clients in their recovery, assists clients in working on identified goals, completes documentation paperwork and contact notes as necessary, and helps clients to obtain necessary services and benefits such as aftercare, housing, continued treatment.

Minimum Requirements

HS Diploma required. Associate’s or BA strongly preferred.

At least one year experience in providing case management within this field preferred.

Must maintain and provide proof of valid driver’s license and automobile insurance in good standing.

Reliable use of personal vehicle as needed.

Responsibilities

Understanding the needs of clients with substance abuse disorders and co-occurring mental health issues.

Providing recovery-oriented services with an emphasis on dignity, respect, and inclusion of family and other natural supports.

Providing clinical case management services including education regarding relapse prevention and symptom management, advocacy, and assistance connecting to vocational, entitlement, housing, and other supports.

Running psychoeducational support groups on a weekly basis.

Completing documentation in a timely manner, including intake paperwork, contact notes, and other required paperwork.

Acting with compassion, integrity, and ethics.
