Overview • $1,500 Sign‑On Bonus • Competitive compensation with flexible scheduling • Visits scheduled based on availability from Sunday‑Saturday, 8 am‑7 pm • Eligible for full-time transition and additional sign‑on bonus
Responsibilities Conduct adult/geriatric assessments including medical history, diagnosis, treatment, health education, physician referrals, case‑management referrals, follow‑up and clear documentation according to Matrix guidelines and protocols. Administer and order point‑of‑care and lab‑based testing as needed. Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate. Collaborate with Primary Care Physician (PCP) on patient education and follow‑up as necessary. Provide services at the member’s place of residence, which can be in their homes or Skilled Nursing Facilities.

Competitive Pay – earn income per assessment completed; monthly average of 20‑100 visits can earn between $1,800 and $11,500. Benefits – mileage reimbursement, 401(k), employee referral bonuses, limited voluntary benefits. Education and Support – comprehensive onboarding training, preceptor assignment, 1:1 feedback, continuing education access. Additional Licensure – support for obtaining additional state licensure and credentialing. Full‑time opportunities – PRN providers can transition to FT roles with a FT sign‑on bonus.
Qualifications Master’s degree or commensurate experience and satisfactory completion of NP licensure. Current NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority as needed. AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty. Current BLS, ACLS, or CPR certification. 1 year of NP experience preferred; new grads encouraged to apply. Valid state driver’s license and proof of adequate automobile insurance coverage. Strong computer skills and familiarity with employee health/medical record software. Excellent verbal and written communication skills.
