" Responsibilities " " " " Conduct adult/geriatric assessments, including medical history, diagnosis, treatment, health education, physician referrals, case‑management referrals, follow‑up, and documentation according to Matrix guidelines and protocols. " " Administer and order point‑of‑care and laboratory testing as needed. " " Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate. " " Work with the Primary Care Physician (PCP) on patient education and follow‑up as necessary. " " Provide services at the member’s place of residence, which may be their home or a Skilled Nursing Facility. " " "
" Benefits " " " " Flexibility – Visits are scheduled based on your availability, from 8 am–7 pm, Sunday–Saturday. You can work during the day, evening, or weekend. " " Competitive Pay – Earn income based on the number of assessments completed. Those completing a monthly average of 20–100 visits can earn $1,800–$11,500 per month, depending on state and assessment rate ($90–$115). " " Benefits – Mileage reimbursement, 401(k), employee referral bonuses, and limited voluntary benefits. " " Education and Support – Comprehensive onboarding, a preceptor, 1:1 feedback, and free access to continuing education. " " Additional Licensure – Business need may support obtaining additional state licensure and credentialing. " Qualifications " " " " Master’s Degree or equivalent experience and satisfactory completion of NP licensure. " " Current NP licensure in state of practice with prescription authority (or ability to obtain prescriptive authority). " " AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty. " " Current BLS, ACLS, or CPR certification. " " 1 year of NP experience preferred; new graduates encouraged to apply. " " Valid state driver’s license and proof of adequate automobile insurance coverage. " " Strong computer skills and familiarity with employee health/medical record software. " " Excellent verbal and written communication skills. " " "
