Position Summary We are seeking a
PRN Physical Therapist (PT)
to join our dedicated in-house therapy team. The Physical Therapist will evaluate, plan, and deliver therapeutic interventions to residents with mobility limitations, functional impairments, or injuries. This role supports both short-term rehabilitation and long-term care residents.
Key Responsibilities
Perform comprehensive physical therapy evaluations to assess strength, balance, mobility, and functional limitations.
Develop and implement individualized treatment plans based on each resident’s clinical needs and goals.
Provide direct therapeutic interventions including gait training, therapeutic exercise, balance activities, and functional mobility training.
Document all evaluations, progress notes, and discharge summaries in accordance with state, federal, and facility standards.
Collaborate with interdisciplinary team members—OT, SLP, nursing, and social services—to ensure coordinated and effective care.
Educate residents, families, and caregivers on safety, mobility strategies, and the home exercise program (HEP).
Participate in care plan meetings and make professional recommendations based on clinical findings.
Follow all regulatory, ethical, and professional guidelines for physical therapy practice.
Qualifications
Doctorate or Bachelor/Master’s degree in Physical Therapy from an accredited program.
Current Physical Therapist license in the state of practice.
Prior experience in a skilled nursing facility (SNF) or similar setting preferred.
Strong assessment, clinical reasoning, and documentation skills.
Ability to work independently and manage a flexible PRN schedule.
