The Larimer County Department of Health and Environment is seeking a Communicable Disease Nurse to perform disease and outbreak investigations and support our Senior Public Health Nurse and Medical Assistant in the Tuberculosis program.

In this role, you will conduct detailed case investigations for reportable conditions, with a primary focus on enteric and vaccine‑preventable diseases. You will also play a key role in responding to and containing outbreaks in high‑priority settings, including schools, childcare centers, and long‑term care facilities.

This position combines investigative work with hands‑on clinical duties to aid in disease mitigation efforts, including specimen collection and vaccine administration. A portion of the workload involves specialized case management for Tuberculosis (TB), where you will coordinate care and monitor treatment plans for patients with active TB disease and latent TB infection in accordance with public health protocols.

The standard work schedule is Monday through Friday, 8:00 AM to 4:30 PM. You may be asked to respond to events outside the typical schedule. During the initial six‑month probationary period, you may telework up to one day per week if business needs allow. After probation, you may telework up to two days per week, depending on business plans and operational needs.

Responsibilities

Perform disease investigations: Receive and review reports of notifiable conditions, interview patients to determine symptom history, exposure sources, and transmission risks.

Lead outbreak response: Investigate and contain clusters of illness in high‑risk settings such as schools, childcare centers, and long‑term care facilities, providing guidance on isolation, exclusion, and environmental cleaning.

Manage tuberculosis caseloads: Coordinate comprehensive care for patients with active TB disease and latent TB infection, monitor medication adherence, assess side effects, conduct home visits, and ensure treatment completion.

Provide clinical services: Perform hands‑on clinical tasks to support investigations and outbreaks, including administering post‑exposure prophylaxis, vaccines, and collecting biological specimens for diagnostic testing.

Documentation and surveillance: Maintain accurate, confidential records of investigations and clinical encounters, entering data into state surveillance systems to track community health trends.

Qualifications

Bachelor’s Degree in Nursing from an accredited college or university (or equivalent combination of education and/or experience) required.

Licensed to practice as a registered nurse (RN) in the State of Colorado required.

One year of experience providing health services as a registered nurse required.

Possession of CPR and First Aid certifications by date of hire required.

Possession of a valid driver’s license by date of hire required.

One year of experience in a nursing program relevant to the role preferred.

What We’re Looking For

Knowledge of disease processes, including transmission modes, incubation periods, and control measures for a variety of communicable diseases.

Investigative interviewing techniques to conduct sensitive, thorough patient interviews and elicit contacts, travel history, and exposure sources.

Infection control in congregate settings, including familiarity with regulatory guidelines and containment strategies.

Clinical proficiency in specimen collection and immunization standards (schedules, storage, and handling).

TB case‑management expertise, including interpreting chest X‑rays and laboratory results, monitoring for drug toxicity, and ensuring adherence over months of therapy.

Risk communication skills to explain complex medical concepts and isolation requirements to diverse audiences.

Supplemental Information Please note: The Department of Health and Environment has a vaccine requirement. Successful candidates will be required to comply with the vaccine requirements or request a waiver.

Employees will receive a 2% market increase on January 8, 2026. Employees who start before January 8, 2026, will be hired at the 2025 pay rate and will receive the 2% market adjustment on that date. Employees who start on or after January 8, 2026, will be hired at the 2026 rate, which includes the market increase.
