Job Summary The Westchester Center for Rehabilitation & Nursing is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our dedicated team of healthcare professionals. Responsibilities
Provide high-quality patient care in accordance with established nursing standards and protocols
Conduct initial and ongoing assessments of patients' physical and emotional needs
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Administer medications and treatments as prescribed by physicians
Monitor and report changes in patient condition to healthcare professionals
Document patient care and progress in a timely and accurate manner
Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
Requirements
Current LPN licensure in the state of New York
Prior experience as a Licensed Practical Nurse (LPN) preferred
Strong communication and interpersonal skills
Ability to work in a fast-paced environment with concurrent priorities
Strong attention to detail and ability to maintain accurate records
Commitment to providing patient-centered care and maintaining confidentiality
