The Licensed Clinical Social Worker (LCSW) will provide individualized counseling, assessment, and therapeutic services to patients in the comfort of their own homes. The ideal candidate will have experience in healthcare, be able to work independently, and have a passion for improving the mental and emotional well‑being of patients with chronic health conditions, elderly populations, and those facing significant life transitions. Responsibilities
Conduct in‑depth psychosocial assessments of patients and their families in home‑care settings to identify emotional, social, and psychological needs. Provide individual and family counseling services, including crisis intervention and supportive therapy. Collaborate with a multidisciplinary healthcare team to develop and implement personalized care plans. Educate patients and families about coping mechanisms, available community resources, and support networks. Advocate for patients’ needs within the home‑care setting, ensuring access to necessary medical, mental health, and community resources. Support and counsel patients with end‑of‑life concerns, including advanced care planning and grief support. Maintain accurate and up‑to‑date patient records in compliance with regulatory standards and confidentiality laws (HIPAA). Provide ongoing support, including follow‑up visits, phone consultations, and coordination with other healthcare providers. Participate in case reviews, interdisciplinary team meetings, and supervision to improve patient outcomes. Maintain knowledge of current therapeutic interventions, social work practices, and healthcare regulations. Qualifications
Education: Master’s degree in Social Work (MSW) from an accredited program. Licensing: Active and unrestricted LCSW license in the state of West Virginia. Experience: Minimum of 3 years of clinical social work experience, preferably in a home‑care or healthcare setting. Skills:
Strong interpersonal and communication skills. Knowledge of crisis intervention techniques and evidence‑based therapeutic interventions. Familiarity with community resources and social service programs. Ability to work independently, manage a caseload, and prioritize tasks effectively. Proficiency in electronic health records (EHR) and documentation standards. Compassionate, empathetic, and committed to patient‑centered care.
