Licensed Clinical Social Worker / LCSW
Rehab Without Walls® Neuro Rehabilitation requires an experienced LCSW to provide counseling and support across all physical, mental, cognitive, and emotional dimensions of recovery in a natural home and community setting.
Overview Make an impact in our Supported Living Program – deliver therapeutic interventions and resource recommendations that address patients’ goals and foster personal, career, and independent living success.
Responsibilities
Guide and counsel individuals and families in crisis, from unemployed adults to children needing foster homes and elderly people lacking care.
Assist clients with obtaining government funds, education, or treatment resources.
Secure and assess medical, psychological, and social data to evaluate circumstances and determine the nature and extent of problems.
Help patients and families through individual or group conferences to plan needs fulfillment and mobilize internal and external resources.
Facilitate behavior modification by increasing self‑understanding of personal issues and clients’ contributions.
Develop and implement service plans aimed at optimal social and health adjustment within patient capacity.
Refer clients to community resources and coordinate with other social service agencies to support community re‑entry or limitations of disability.
Prepare and maintain patient histories, service plans, and reports, and conduct follow‑ups to assess service quality and case status.
Maintain client records in compliance with organizational objectives and external regulatory guidelines.
Perform other duties as assigned.
Qualifications
Master’s Degree in Social Work.
Five or more years of clinical social work experience.
Valid and current Licensed Clinical Social Worker (LCSW) or Licensed Medical Social Worker license.
Ability to conduct psycho‑social assessments and formulate viable care plans.
