Treatment Coordinator - Behavioral Health (West Phoenix) Join the West Valley Behavioral Health Center as a Treatment Coordinator. You will provide ongoing care‑coordination services to assist clients and families in addressing their primary care and behavioral health needs, facilitating community support, identifying resources, and resolving barriers. This position is a vital part of the behavioral health continuum.

Responsibilities

Perform ongoing coordination of services to children and adults in accordance with the treatment plan.

Respond to initial crisis calls and identify needs & necessary resources accordingly.

Use established best practices to engage formal and informal resources for the client, including transition, discharge, and aftercare plans of behavioral‑health services while demonstrating a positive, strengths‑based attitude of teamwork and shared leadership.

Provide coordination of behavioral‑health services with other agencies, such as Department of Child Safety (DCS), Probation/Parole Departments, Division of Developmental Disabilities (DDD), Arizona Long‑Term Care, schools, and other providers.

Qualifications

Bachelor's Degree in behavioral health field preferred; a combination of experience and education may be considered.

Relevant experience working in a behavioral‑health setting.

Level 1 Fingerprint Clearance Card.

Verification of education, employment, references.

MVR matching JFCS driving guidelines.

AZ Driver's License and Valid Auto Insurance.
