Treatment Coordinator - Behavioral Health (West Valley) Jewish Family & Children's Service of Arizona

Pay Range:

$21.00/hr - $23.00/hr

(Base). Actual pay will be based on skills and experience.

The Treatment Coordinator provides ongoing care coordination services to assist in addressing the client and family’s ongoing service needs, including both primary care and behavioral health. The position assists clients and families in obtaining community support, identifying resources, and resolving barriers.

Responsibilities

Performs ongoing coordination of services to children and adults in accordance with the treatment plan.

Responds to initial crisis calls and identifies needs & necessary resources accordingly.

Uses established best practices to engage formal and informal resources for the client, including the transition, discharge, and aftercare plans of behavioral-health services while demonstrating a positive, strengths‑based attitude of teamwork and shared leadership.

Provides coordination of behavioral‑health services with other agencies, such as Department of Child Safety (DCS), Probation/Parole Departments, Division of Developmental Disabilities (DDD), Arizona Long Term Care, schools, and other providers.

Qualifications

Bachelor's degree in a human services field preferred; a combination of experience and education may be considered.

Relevant experience working in a behavioral‑health setting.

Verification of education, employment, references.

MVR matching JFCS driving guidelines.

Arizona driver’s license and valid auto insurance.
