Medix is now hiring a full time Mental Health Therapist for a senior-focused, neighborhood-centric, multidisciplinary primary care organization dedicated to addressing the physical, behavioral, social, and cultural needs of underserved Medicare-eligible Hispanic populations. Designed with a multigenerational approach, this company strives to deliver on the quadruple aim—improving patient outcomes, enhancing patient and employee experience, and reducing the total cost of care.
Pay : $80,000 - $105,000
Shift : Mon-Fri (8am -5pm), Onsite
Location : Phoenix, AZ 85018
Setting : embedded in a primary care clinic
Population : Geriatric, mild to moderate - client matching provided
Required :
Must be bilingual English/Spanish
Must hold one of the following licenses:
LMFT, LCSW, LPC
Masters Degree required (Social Work, Psychology, Marriage and Family Therapy, or Mental Health Counseling)
Responsibilities :
Conduct patient assessments, develop treatment goals, plan therapies, and evaluate progress.
Maintain thorough, confidential clinical documentation and communicate relevant social and psychological information to providers.
Coordinate care with patients, families, providers, and community agencies to ensure collaborative, comprehensive support.
Offer social services and counseling related to social, psychological, financial, and family issues.
Assist with direct patient care and provide clinical consultation to staff, particularly in areas of trauma, personality disorders, geriatric care, and substance use.
Lead or support educational programs, staff training, and community outreach on behavioral health and wellness.
Build and maintain relationships with community organizations and develop new resources to meet patient needs.
Review provider cases, support care planning—especially for home-based needs—and perform other duties as assigned.
