About the Role During a visit that can last up to one hour, providers review and observe a member’s current health, medical history, medication adherence, social environment, and other risks. This in‑depth assessment provides insight that is difficult to capture in routine office visits and helps close care gaps in a single encounter.

Responsibilities

Conduct adult/geriatric assessments that include medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow‑up, and clear documentation per Matrix guidelines and protocols.

Administer and order point‑of‑care and lab‑based testing as needed.

Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.

Collaborate with the Primary Care Physician (PCP) on patient education and follow‑up as necessary.

Provide services at the member’s place of residence, which can be in private homes or Skilled Nursing Facilities.

Flexibility: Visits are scheduled based on YOUR availability. We conduct assessments Sunday – Saturday from 8 am – 7 pm. You can work during the day, evening, and/or weekend based on your schedule.

Competitive Pay: Earn income based on the number of assessments you complete. Monthly totals range from $1,800 to $11,500, depending on your state and the number of visits.

Additional Licensure Support: Support to obtain state licensure and credentialing in neighboring states if business needs arise.

Full‑time Opportunities: PRN providers can transition to FT roles with a sign‑on bonus.

Qualifications

Master’s degree or commensurate experience and satisfactory completion of NP licensure.

Current NP licensure in the state of practice, including prescription authority or ability to obtain prescriptive authority.

AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.

Current BLS, ACLS, or CPR certification.

1 year of NP experience preferred; new graduates encouraged to apply.

Valid state driver’s license and proof of adequate automobile insurance for the state of residence.

Strong computer skills and familiarity with employee health/medical record software.

Excellent verbal and written communication skills.
