Conduct thorough physical assessments and review medical histories to evaluate patients' conditions accurately. Create and tailor treatment plans that align with each patient's unique rehabilitation goals and needs. Implement therapeutic exercises and training programs to enhance mobility and functionality. Educate patients and their families on injury prevention, wellness strategies, and recovery processes. Monitor and document patient progress toward rehabilitation goals, adjusting treatment plans as necessary. Collaborate with a multidisciplinary team of healthcare professionals to ensure comprehensive patient care. Maintain detailed patient records and documentation in accordance with regulatory standards. Qualifications
Master’s degree in Physical Therapy. Minimum of 1 year of experience as a physical therapist in a home health setting. Valid state licensure to practice as a physical therapist, adhering to professional standards. Current CPR certification required upon employment.
