Compensation Range
$23/hr - $26/hr, commensurate with experience. Benefits
Three comprehensive healthcare plans available. Housing Works pays the majority of the plan cost. PTO accrues immediately, up to 30 days earned in the first year. An educational benefit covers tuition loan reimbursement, tuition costs, and textbook expenses. Overview
Scatter-site Health and Housing Project (SHIP) seeks a Residential Case Manager to deliver in‑home support services to singles and families, promoting independent living within a supportive housing program. The role includes intensive case management, counseling, health monitoring and appropriate referrals, enhancing clients’ life skills and preparing them for permanent independent housing upon graduation. Responsibilities
Manage a caseload of 24 individuals. Maintain program contractual compliance with individualized treatment plans, assessments, progress notes, and other required tools, ensuring confidentiality. Adhere to service delivery requirements in contracts. Submit required reports on time. Conduct housing meetings for clients in shared apartments. Maintain regular contact with clients, including monthly home visits. Follow agency standards and policies. Monitor residents’ medical conditions, including viral suppression, and refer as needed. Assist new residents with apartment move‑in and setup. Provide referrals for substance use, nutrition, clothing, mental health, and dental; refer internally when practical and externally as needed. Keep orderly and organized charts. Provide crisis intervention and mediation as necessary. Serve as backup to other case managers when necessary. Coordinate health, mental health and substance abuse services, utilizing internal and external resources to meet chronically homeless clients’ needs. Coordinate multiple enrollments of clients in other Bailey House/Housing Works programs. Obtain updated medical and mental health documentation for each client. Assist clients with budgeting, shopping, rent arrears, ADL training, and orientation to new clients with program structure. Attend weekly supervision, team meetings, and trainings. Prepare for supervision with an agenda discussing casework challenges and successes. Travel with clients or by themselves using mass transit. Advocate on behalf of clients with strong communication skills. Contribute to team effort by accomplishing related results as needed. Participate in planning and execution of client activities, as determined by program administrators. Maintain agency/program operations and workflow. Follow agency policies and procedures. Maintain professional and technical knowledge. Understand and comply with agency guidelines.
